Parishioner Information Sheet
Every household that will be photographed and even those who are not desiring to be photographed for the directory is encouraged to fill out a Parishioner Information Sheet. (IMPORTANT: Please only fill out the sheets supplied by the church on Sunday.)
Please note: This project is designed to create a way for the parish to get to know one another by putting names and faces together. It is also an effort to update and refresh the overall parish database/directory.
Some information it would be helpful to research at home prior to filling out the form: this information may or may not be on record and we will gladly check the Parish register if you need assistance or are missing this documentation.
- Baptismal dates and places if you know them for each member of your household
- Confirmation dates, place, and Bishop if you know them for each member of your household
Working together to include everyone…
Our goal is to photograph every household that is part of the Cathedral Community. It is important to understand that we will do our best to get to everyone at your preferred time; however, the time for each session is short. We ask that you would be patient with this process if you are asked to wait to a subsequent Sunday. While you are preparing in Wellbrock Hall there will be activities for children and refreshments as usual. The photographs will be taken in the playground- the play area and lawn will have limited availability as a result. (The Portico will be used in the event of rain.) Home visits will be made to those parishioners who would like to be included, but for whom mobility is limited. Mirrors and baby wipes will be on hand for touch ups prior to your photograph. Step 1 : Choose a time
Choose a time for your family to be photographed.
Step 2: Household Details
Gather in Wellbrock Hall or at the tables in the courtyard. There will be Parishioner Information Sheets available with pens for you to begin completing. When you are finished or nearly finished (from Wellbrock) go to the desk in the Lobby to receive a number. (from the courtyard tables) a number will also be issued.
Step 3: Gather your household
When your number is called simply go to the Parish House entrance and the volunteer will take you to the photograph waiting area near the fountain.
Step 4: Say “Cheese” & Finish
Take you family photo and turn in your Parishioner Information form (if it is not complete -simply finish filling it in at this time or let the member of our project team know which areas you have left blank.)
Step 5: Order Your Own Pictorial Directory
Later this year you will be able to order your own Pictorial Directory and access the information in a secure manner via the web. (more details on this when the project is completed.)
